SMART
is the Streatham Mainliners Assessment Referral Team, established in 1999, as a
direct-access service for the growing number of people with drug related issues
in the South Lambeth area of London. The service is funded by Lambeth Primary Care Trust to provide information and advice on
harm reduction, safer sex practices, safer injecting and HIV/HCV prevention.
In addition, to the full
menu of Mainliners services, SMART also provides substitute opiate prescribing
through shared care with GPs. Each client is allocated a key worker who offers
support and advice around drug use, harm reduction and relapse prevention. More details can be found on our service leaflet.
SMART was recently awarded a whole series of green flags by the National Treatment Agency following an extensive evaluation process and grading.
The four key
service objectives of the SMART Project are:
1. To
reduce drug-related harm through providing a range of services for people
affected by, or at risk from, HIV, Hepatitis C and other blood-borne viruses.
2. To
reduce harm through working with and supporting individual clients, providing
choices for individuals who want to stabilise, reduce or achieve a drug free
status.
3. To
provide a holistic approach to meeting client needs by working in partnership
with other professionals and agencies and, as appropriate, by referring service
users to other providers.
4. To
offer comprehensive service to empower and enhance client health, through direct
service
As a street agency SMART
offers a range of information, advice and interventions to improve the quality
of life for clients. Services are free, non-judgmental and confidential.
Our Approach
SMART is client led and is underpinned with a Psycho-social approach. We aim to enable clients to stabilise not only in their drug use but to find stability in all areas of their lives.
We try to provide consistency for clients through ensuring they have the same keyworker throughout their time at SMART.
Goals are identified through tailored care plans where the client takes an active part in drawing up the care plan and it is reviewed on a regular basis.
